Unusual Presentation of Traumatic Neuroma on the Gingiva Following Road Traffic Accident

Introduction: Traumatic neuroma (TN) is a reactive non-neoplastic proliferation of nerve tissue that results from injury to a nerve, usually after a surgical procedure. The lesion is uncommon in the oral cavity. The mental nerve region, lower lip, and tongue are the most common sites in the oral cavity where the lesion may occur. Few cases of TN have been described on the palate. However, it is uncommon to be found in the gingiva. To our knowledge, there have been no previously published reports of a TN of the mental nerve in the region of the gingiva with atypical clinical appearance.
Case Presentation: 46 years old male came to the Clinic of Periodontics for a routine dental examination. The patient reported that he had been involved in a traffic accident 27 years ago. The patient had been hospitalized for 7 days to treat a dentoalveolar fracture. Gradually, the patient began to notice an altera-tion of sensation in his lower lip but he did not receive treatment, because he used to it. Upon presentation, an intraoral examination revealed a longitudinal, firm, hyperplastic soft tissue swelling covered by normal mucosa. The lesion was located between the mandibular right canine and the first premolar. The patient underwent excisional biopsy. Histopathological examination confirmed the diagnosis of TN. After one week follow up, the healing process improved, and sutures were removed. It was noted at the four month follow up that there was no recurrence of lesion, no alteration of sensation, and patient was completely asymptomatic.
Conclusion: Complete excision of TN with careful monitoring can be a successful treatment for TN in or-der to abolish the symptoms and avoid recurrence of the lesion
