CLINICAL EXPERIENCES

 

Cardio Thoracic Surgery:

 

Currently, I am working as Consultant & Assistant Professor (Surgery) in the Thoracic surgical unit in King Khalid University Hospital, which is considered to be among the largest hospitals in the kingdom.  It is a teaching hospital attached to the medical college, and it provides all kinds of Medical services to the patients, it has a capacity of 870 beds.

 

Working previously in this job as the only Senior Registrar & Honorary Clinical Teacher (Surgery), with two Consultants (and on call one in two), I gained a lot of experience in the clinical teaching and the academic field which my job involves 2 days per week doing out-patient clinics, 2 days per week in theatre doing or assisting in major surgery.  Two clinical sessions per week of teaching thoracic surgery to the third, fourth year and the final year medical students.

 

I have gained a lot of experience in the surgical management of the inflammatory lung disease and their complications e.g. (T.B., bronchiectasis, aspergilloma etc.).  Also assessing the patients pre-operatively for lung transplantation.

 

My previous experience in the U.K. included working as a senior Registrar in the Thoracic firm at the Norfolk and Norwich Hospital in Norwich, where I was the only Registrar for two consultants, Mr. Ross and Mr. Vaughan for 2 years, I continued my training in cardio thoracic surgery at the Cardio thoracic Centre, which is the largest centre in this specialty in the U.K.  I have gained good experience in both cardiac and thoracic surgery.  My thoracic surgical experience includes performing, with direct supervision, all major operations like oesophagectomies (16) and I have been first assistant at (113) gastro-oesophagectomies, I have performed (P) and assisted (A) with the following operations.

I have performed also the common thoracic procedures like lung wedge resection (69), open (44) and VAT lung biopsies (97).  VAT pleurodesis, pleurectomy and pleural biopsy (68), lymph node biopsy, tracheotomies, mediastinoscopies and mediastinotomies (62), thoracoscopies and laparoscopies (92).

 

I have gained good experience in performing flexible and rigid bronchoscopies (129), fibroptic and rigid endoscopies, which include pharyngoscopy, oesophagospy, gastroscopy under G.A. and sedation.

 

I have performed 420 endoscopies under G.A. or sedation including gastroscopy, oesophagoscopy and dilatation for therapeutic or diagnostic purposes like dilatation for benign and malignant strictures, which are either fibroptic, or rigid using celestin dilatation, Maloney Dilatation or Balloon Dilatation.  Some of these endoscopies are for insertion of oesophageal prostheses like the Atkinson tube, Wilson Cook tube or insertion of markers for celestin DXT, or using lasers for treatment of oesophageal or bronchial carcinoma.

 

I have gained good experience in handling major thoracic trauma and emergencies including assisting or performing in hospital emergency thoracotomies, sternotomies for major trauma haemorrhage or gunshot injury.

 

My cardiac surgical experience includes assisting as a first assistant and performing part of coronary artery bypass grafting operations, as well as both aortic and mitral valve replacement under supervision.  I frequently open and close most of the heart surgical procedures and harvest the internal mammary artery and put the patient on bypass.

 

I have assisted at many more complex procedures including LV aneurysmectomy, repair of VSD post infarct, aortic dissection and ascending aorta replacement with deep hypothermia and circulatory arrest.

 

I have gained useful exposure to many different techniques of myocardial protection including the use of cold crystalloid and blood cardioplegia, intermittent cross clamping, continuous coronary perfusion and continuous warm hyperkalaemic cardioplegia.

 

I have gained very good experience in managing patients following elective and emergency coronary artery bypass surgery and valve replacement procedures, LV aneurysmectomy, repair of post infarct VSD, aortic dissection, patients with cardiogenic shock in IABP, acute lung injury following pulmonary resection, oesophageal resection and septicaemic patients in Intensive Care and High dependency Units, looking after immediate post-operative patients after having cardiac or major thoracic operations.

 

During my on-call in the CTC, I was first on call for the 18 beds in the Surgical Intensive Care Unit, where I have inserted and removed intra aortic balloon pumps (IABP), Swans Gins catheters and central venous lines.

 

During all my post in cardio thoracic surgery, I have gained valuable experience in assessing new and review cardiac and thoracic patients at our outpatient clinics.  I regularly attend the weekly chest and cardiac meetings and give presentations, or present the morbidity and mortality in cardio thoracic surgery for local surgical post-graduate education and audit meetings.  I was involved in teaching of final year medical students as the centre provides comprehensive facilities for the teaching of all final year undergraduates of the University of Liverpool and in the previous job at Cambridge Medical School.  Currently, teaching the fourth and final year medical students clinical sessions twice a week in King Khalid University Hospital.
